When participating in physical activity of any type, I acknowledge there is a possibility of injury (ranging from minor to catastrophic and/or fatal). When engaging in any of the prescribed workouts or exercises provided by Andrew Evans and/or AZ Endurance LLC, I agree that I do so at my own risk, my participation is voluntary, and I assume all risk of injury to myself.

I also agree to INDEMNIFY AND HOLD HARMLESS Andrew Evans and/or AZ Endurance LLC, from and against any and all losses, liabilities, obligations, damages, Claims, judgments, assessments, penalties, costs and expenses, including attorneys’ and other professionals’ fees and disbursements based upon, attributable to, resulting from, or in any way related to, the breach or violation of any agreement, covenant, obligation or other provision set forth herein, or based upon, attributable to, resulting from, or in any way related to, my involvement in Andrew Evans and/or AZ Endurance coaching programs.

I acknowledge that Andrew Evans is not a medical doctor and that advice and recommendations provided are based upon Andrew Evans' training and experience as a runner. Additionally, I agree and understand that I am solely responsible for my health and medical condition, and that it is my sole responsibility to determine my physical and medical fitness to undertake a strenuous training program.

The monthly coaching plan is offered on a month-to-month basis with no long-term contract. You may cancel (or pause) at any time. Once you have paid for a month, it is not refundable.

I confirm that I am of adult age and I execute this document with full knowledge of the contents and consequences stated in this Agreement.
